Guide to Immunization Requirements in Oklahoma
- The Oklahoma State Department of Health’s Immunization Service Program has launched a new online portal for parents and guardians to submit immunization exemption requests online. More information can be found in PDF and the online portal for exemption may be accessed at https://oklahoma.gov/health/immunizations.html . All exemptions will expire at the end of the student’s completion of 6th grade. Parents/guardians will need to reapply prior to the start of the student’s 7th grade year.
